Luis said he was backing the forklift out of aisle 12 when the rear wheel caught the corner of the pallet rack. He said Kevin was about thirty feet behind him, picking, and jumped back when the boxes fell. Luis said the backup alarm was not working and that the left mirror had been cracked since Monday. Luis estimated his speed at about walking speed. Luis admitted he had been looking at his phone while driving. The rack upright was bent about two inches, and aisle 12 was taped off until maintenance could check it. Luis was taken off the forklift for the rest of the shift pending the review, which I told him was standard and not discipline. Luis has had two prior forklift incidents this year.
