[00:08] SUPERVISOR MOSS: Luis, can you walk me through what happened?
[00:11] LUIS: I was backing the forklift out of aisle 12 and the rear wheel caught the corner of the pallet rack.
[00:18] SUPERVISOR MOSS: Was anyone in the aisle?
[00:20] LUIS: Kev was about ten feet behind me, picking. He jumped back when the boxes fell.
[00:26] SUPERVISOR MOSS: Did anything hit him?
[00:28] LUIS: One box clipped his shoulder. He said he was fine.
[00:32] SUPERVISOR MOSS: Was your backup alarm working?
[00:35] LUIS: It was. But the mirror on the left side has been cracked since Monday. I put it on the checklist.
[00:42] SUPERVISOR MOSS: How fast were you going?
[00:44] LUIS: Walking speed, maybe.
[00:47] SUPERVISOR MOSS: Okay. The rack upright is bent about two inches. We're taping off aisle 12 until maintenance checks it.
[00:55] SUPERVISOR MOSS: Kevin, how's your shoulder?
[00:57] KEVIN: A bit sore. I don't need to go to the clinic.
[01:01] SUPERVISOR MOSS: I'm noting that you declined the clinic. Tell me if it gets worse.
[01:06] SUPERVISOR MOSS: Luis, you're off the forklift for the rest of the shift pending the review. That's standard, not discipline.
